With 22 minutes left, Hojbjerg scored the opening goal for Spurs after being the first to react to a loose ball on the edge of the penalty area and firing a shot into the bottom-right corner of Edouard Mendy's net.
But when Sterling's pass was met by James with a brilliant side-foot finish, Chelsea promptly regained the initiative and reclaimed the lead, sending Thomas Tuchel running down the touchline in celebration.
When Spurs won a corner in stoppage time, though, Kane's skillful header took a little deflection from James before nestled in the far bottom corner, setting off raucous celebrations from the visitors.
